Become a New Patient
***Please note that you do not need a Physician Referral to book a new patient appointment. It is advisable to consult your insurance provider to determine whether your insurance plan mandates a referral for specialist visits. If a referral is required, it is your responsibility to obtain one. Failure to do so may result in you being responsible for the visit fee in the event that your insurance denies payment due to a lack of referral from your primary care provider.***
We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:
- Make an Appointment (via phone or our website)
- Sign up for our patient portal
- Download your patient forms online through the patient portal
Before your appointment, we will send you a link to add your medical history and medication list and upload your identification and insurance card. You must sign the new patient forms and pay your co-pay. When you come to our office as a new patient, we’ll ask you to complete some initial forms if you have not only the forms before the appointment. To ensure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.Remember to bring:
- Your insurance card
- Valid photo ID
- List of current medications
- Office co-pay
- Credit Card
In an effort to respect the time of all the patients, our staff strives to stay on schedule so that other patients do not have to wait.For patients who are delayed and arrive late for appointments, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled
Please be advised that our facility does not provide medical services to individuals under the age of 18 years
